Reading about a former nurse at a Lexington, Kentucky VA Medical Center Home accused of murder as the same VA center stands accused of fostering conditions leading to "elevated mortality rates," it’s easy to miss details of these conditions.
Jennifer Hewlett of the Lexington Herald-Leader (Kentucky) brings to light a consulting firm’s scathing September 2009 report that says multiple failures at the Lexington VA Home contributed to higher-than-expected mortality rates.

Among other things, the consulting firm found instances of emergency department triage being performed by a desk clerk; nurses not being able to readily identify or contact on-call residents, especially on nights and weekends; and doctors’ orders not being followed.
